Custom made for me by my best friend, "DWB." I've known him for 51 years, and in that whole time, his hobby has been carving leather. He's also made guitar straps for me, but again, that would break the rules here. He's also rebound Bibles for me over the years, so take your pick...
All three of these came through our house fire, with varying degrees of smoke and water damage, and that was 10 years ago, so they're a little worse for the wear now.

An automatic watch with seven day reserve. Not fancy, just built to give a 10-beat sweep and <1 second per week deviation. Easy to read, day and date, lume to the hands and markers, resistant to day to day living. Understated elegance.
Keyboard. There are very few that I like, and the spacing and arrow keys are wrong on most.
Mine would be a double headed hammer. Not one on each side, but twin heads on one end. One for the nail and one for my thumb.
Felt tip pens, I use those things daily and always find something that I would change for ease of use of the things.
I've always wanted Towel Pants.
I would have my own line of pants made out of fluffy towel stuff and maybe some furry rug material.
But also a pair of softshell hiking pants, with the right pocket depth, carefully designed reinforcements, top quality zips, etc.
How about ski goggles that all look like old flying or military/biker goggles? I like odd ski goggles. Goggles and caps.

With a drawstring waist? Excellent! Those could be my new "eatin" pants! Adjustable room for my expanding gut and you can clean your hands on the towel material!
I think mine would be a hand-forged knife and axe set. A 3" bladed non-locking folder, a 6" bladed fixed-blade (both in D2) with Micarta scales, a 12" camp hatchet with a 1lb head (in 1095), a 20" forest axe with a 2lb head (1095 again) and a 36" felling axe with a 7lb head (yup, you guessed it, in 1095). All would have leather sheaths / masks.
