I know most come from Fender with extra lights/9s. I would guess most nuts can accommodate lights/10s without any issues except for a small setup adjustment possibly.

When does the factory nut slot become a problem requiring modification for heavier string sets? 11s? 12s?

I used 11s for many years on Fenders without any problems. With 12s you might need to widen some of the nut slots.

New nuts should be part of every setup. In a perfect world. By all means every time you change your strings' sizes.
