Tele: 3-Way with two push-pull pots - Series/Parallel and Phase?

    Hey all. Sorry if this question has already been asked, but I'm looking to wire a Tele with a 3-way switch and two push/pull pots - the first would toggle between series and parallel, and the second would toggle between in/half out of phase. I know this would only be engaged when the pickup selector is in the middle (both pickups). I've seen this done with a 4-way switch and a single push/pull pot, but would prefer a 3-way switch, since when playing live, I don't want to stumble around with a four-way switch (if that makes sense). Thanks to anyone who may have a link to a diagram - I've looked, but so far no luck.

    Why not just put in a 5-way superswitch? Bit of a pain with a standard control route but easier and doesn't affect sound in the traditional position.
