I was changing strings on my vintage hot rod 52, and one of the little metal grommets fell out of the back of the guitar. Is that common?

No but it can happen. Ideally those go in snug. Those are called ferrules and it seems like theres a mismatch between barrel and hole size. Do you have a caliper to measure?

It's happened to me and it's annoying as hell.
    Still have the ferrule?

I think you can also put some clear nail polish on them to prevent them from moving.
