I was going to ask that same thing this morning! I never could get a good look at it. And I didn't think about it at the time, but I have a DVR, I'm just not to the point where I always remember I can pause and rewind...
It looked like the top had a regular tele setup, bridge and neck pickup. I don't think the bottom had a neck pickup, and I never got a good look at the bridge to see what type it was, or what kind of pickup was in it.

I saw that on TV couldn't believe it; I ignored the bONJovi performance in favor of figuring it out -- just as well, huh??? :) They changed keys between songs and one is capo'd so since it's not a 6 vs. 12 situation ala Page, I'm guessing he did it for sheer vanity/ease of changing "guitars" between songs. Maybe one was in open D or something the other regular.

Stage vanity, of course; but it looks played
I saw the double-neck last night and just knew a thread would pop up here.
Any double-neck is primarily for stage and show, but it seemes like the boards on both necks of Sambora's have some decent wear, making me think that this one gets played on a regular basis -- not something pulled out just for the AMA TV appearance.

I think they were different scales. (ie baritone/standard)

The show played more than once on the Armed Forces Network (aren't we lucky), so I had another look. It looks to me like it is indeed an Esquire set up on the bottom neck. It looks to me like the top neck is a "standard" Tele. The apparent difference in the neck lengths is due to the necks placement in the cutout on the body.

Warmoth Body
Hmmmm.....standard neck on the bottom for regular playing.....short-scale neck on the top for open tuning and slide......I prefer having the guitar up higher for slide, anyway.

Definately a cool looking guitar. It is not a baritone neck\regular scale setup though. The top neck looks longer, but notice the bridge position. So, guess it is just a standard scale double neck. As far as the wear on the neck, I bet ya it is a custom shop relic job, just my guess. Definately a cool guitar, but would be even cooler if that top neck was a baritone....

wow heavy
Its a back breaker for sure. Its cool and all but without the 12 string not worth the extra weight no matter how cool it looks. I own a 6/12 Gibson and its a monster, monster heavy that is. I was amazed how heavy it was after it arrived, I bought it via internet before I played it. Jimmy Page was super human to fling his around as long as he did I'll tell ya. I'm 6' and 240# and mine had me wincing in about 15 minutes.
