5 way super switch for Tele

has anyone seen a diagram for a 5 way super switch for a Tele that gives the following combinations?
1 Neck
2 both in parallel
3 bridge
4 both in series half out of phase ( or just out of phase)
5 both in series
I had my thinline wired this way and love the way it works but have lost the diagram. I've spent hours on the net looking at wiring schemes but cannot find this again. thanks for any help. adios d

thanks Garrett, I've copied these already. it's very embarrassing to lose a diagram like that. I F-disked my computer a number of months back and must have forgotten to add the diagram to my must save list. even checked the backup drive where I stashed all my must save files, no luck. The best part of the five way setup that I have for my thinline is how natural it is. you go through the normal three positions then the last two hot ones. I may have to try and reverse engineer it from looking at the wiring on my thinline. can't understand why I can't find it on the internet, I found it somewhere before, but now it's gone.

I've been combing the web the last couple weeks for super switch diagrams and haven't seen anything with the positions you want in that order. I haven't found much for two singles at all, for that matter.

You can definitely mod Deaf Eddie's above diagram to get what you want. It just takes a bit of knowledge to be able to move things around to the order you prefer.
