Bridge Chords for Soul Man

Hello...I'm having a hard time figuring out the chords at the end of the bridge of Soul Man.....The chords I have so far are Eb....Bb...C....and then I'm not finding them. I searched some sites and some said C# but that didn't sound correct. Then a couple others said C/D and then D/Eb but I'm not sure what chords those are. Any help would be appreciated...

Yes, it's C/D - a C with a D on the bottom.

The chords are Eb, Bb, C, C/D, C#/Eb... try this...

You're welcome dparel... it's such a useful chord... pops up in everything from the Beach Boys to Big Star to Teenage Fanclub... also in The Stylistics' 'Stop, Look, Listen' – I'm loving all that cheesy '70s soul at the moment, such great progressions... the other killer altered-root soul chord you need is Am/F#...
