what chord is this?

its like an D chord with the 5th lowers one fret. the notes are F# D G#

If it is a D root it is a D add #11. Or D B5. But put it in the context of a progression and it might make for sense to call it something else.

Leon is correct. Many times when an "altered" chord is being played, the name and function of the chord depends on the context. If the chord were in the V position with the bass playing a D, we have a Vb5 which is a very common move from V to 1. More common would be a D7b5 which would consist of D F# Ab(G#) and C. If the chord were used as a I chord it would be a D with #11. Usually there would be additional notes in the chord, perhaps a M7(C#). Voicing and context become important.

In the world of altered chords and upper partials, all is not cut and dried.
