Making a Fender style Tube Chart

Does anyone have a template, .jpg, .gif or .bmp file they use to customize and print tube charts? I know that I can create one myself - just looking for a shortcut here. Something that looks Fender-ish with the format, and graphics to start out with. Post 'em if you got 'em....Gracias
