the index finger is not released, but it does "pivot." when the middle goes for the F#, the knuckle on the index finger is pointing more toward the nut. when the pinky extends for the A, the finger pivots and the knuckle is more parallel to the fret...but i never need to release pressure...

as for picking, straight up alternate, though i must admit, my first instrinct was to treat every F# to open E and A to open E as a pull off, and i played those with my picking hand middle finger, hybrid style...then the speed really took off, but that might not be the sound you want...
