Ray e-mailed this question to me, and I thought I'd go ahead and post my response, just to keep everybody in the loop. OR, maybe someone else will have a brilliant solution using hardware/techniques that I am not aware of - I can still learn new tricks, ya know!
The short answer is: AFAIK, no, it can't be done adding a SINGLE toggle or p/p switch.
You COULD wire up a Superswitch to get the second list of tones, but there's no single switch you could add to that scheme that would give you the ability to go back and forth between the two tone lists that he wants.
My "best" idea is that you could add TWO p/p pots and ALMOST get there:
With one p/p to change the neck and bridge pups to SERIES with the mid pup, that would get you the second list's #1 and #5 tones. Pulled, throws #2, #3, and #4 would just be the mid pup only, redundant...
With a second p/p for "neck-always-on" you could get the #2 and #3 tones (tone #3 would be at #1 throw on the 5-way).
Pulling BOTH at the same time would make throw #1 bridge and neck parallel, both in series with mid, with the redundant #2, #3 and #4 mid pup-only tones. Throw #5 just would be neck-series-mid.
The scheme doesn't get his #4 throw, though. The other issue with that idea is the S-D 59er he has in the neck with its braided-shield lead - that pup is a NO-GO for any series mods where it is the "second" pickup. He'd need to use something else in the neck, or mod the pickup.
There are other types of switches that CAN accommodate his request, but they would require a bit of a rethink to the standard Tele control layout.
